Options for Meeting these Requirements
1.Physical Exams
- Exam by your private doctor: After your child's exam by your doctor, have the doctor complete the required Physical Exam Form and return it to the school nurse. Form available from the school nurse, or on the district website.
- Exam by the school doctor: The required physical exam may be done by the school doctor, with written parent permission. There is no charge for this service. Permission forms will be sent home when the date for these exams has been scheduled with the doctor.
2.Dental Exams
- Exam by your private dentist: After your child's exam by your dentist, have the dentist complete the required Dental Exam Form and return it to the school nurse. Forms are available from the school nurse, or on the district website.
- SMILE PROGRAMS...the mobile dentist program: The required dental exam may be done by a licensed dentist with this in-school program, with the completed parent permission form. The mobile dentist program performs exams, x-rays, cleaning, fluoride, and sealants when appropriate, during school hours. Registration/parent permission is required for this service. Children will not be seen without proper registration. Insured and un-insured children age 3 and up may be scheduled. No child is turned away based on ability to pay. Grants are available.
