All students in Pennsylvania are required to furnish documentation of their immunization record as required by the Pennsylvania Department of Health. Documentation must be provided for the following immunizations, which are required for all students entering school:

4 properly spaced doses of Diphtheria and Tetanus. 1 dose must have been given on or after the 4th birthday.

3 properly spaced doses of the Polio Vaccine.

2 doses of the MMR (Measles, Mumps, Rubella). The 1st dose must have been given on or after the 1st birthday.

3 properly spaced doses of Hepatitis B. The 1st and 2nd doses must be 28 days apart. Dose 3 must be at least 2 months after the 2nd dose and at least 4 months after the 1st dose. Dose 3 must also have been given on or after age 6 months.

2 doses of Varicella (chickenpox) vaccine given on or after the 1st birthday OR documentation of the child having had chickenpox disease.


For those who do not have insurance that will cover vaccines, vaccines can be administered free of charge or for a minimal fee at the State Health Department on North George Street on Wednesdays from 9-11 am and 1-3 pm. An appointment is required. A parent must accompany the child and you must bring the child's immunization record with you. Call 771-4505 for further information.
